Nurse Practitioner

Buena Vista Hospice is seeking a Hospice Nurse Practitioner to provide high-quality hospice and palliative care to patients with serious and terminal illnesses across Ventura and Los Angeles Counties. In this role, you will work collaboratively with the Hospice Medical Director and an interdisciplinary team to conduct patient assessments, manage symptoms, perform hospice eligibility evaluations, and develop individualized plans of care that support patients and families during end-of-life care.

Responsibilities

• Conduct comprehensive patient assessments, including history and physical exams
• Perform required hospice face-to-face visits and evaluate eligibility for hospice certification and recertification
• Develop and manage individualized treatment plans
• Prescribe medications and treatments in accordance with state regulations
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care
• Manage pain and symptom control to improve patient comfort and quality of life
• Educate patients, families, and caregivers on disease progression, symptom management, and end-of-life care
• Complete timely and accurate clinical documentation
• Ensure compliance with Medicare, Medicaid, HIPAA, and hospice regulatory requirements

Required Qualifications

• Active and unrestricted Nurse Practitioner (NP) license
• Master's degree in Nursing from an accredited program
• Current CPR certification
• Minimum 1 year of Nurse Practitioner experience
• Valid driver's license, reliable transportation, and auto insurance

Preferred Qualifications

• Minimum 1 year of hospice or palliative care experience

Schedule and Employment

• Per Diem position
• Pay Range: $50.00 - $65.00 per hour
