Nurse Practitioner, Bridging Care Provider

ChenMed is seeking a Nurse Practitioner to serve as a Bridging Care Provider, working within our clinical operations team to deliver direct patient care in medical centers, virtually, and in patients' homes. You will work under the guidance of the Regional Chief Clinical Officer, training new primary care providers on our technology and EMR systems while providing clinical coverage in understaffed centers and addressing patient care needs.

This role focuses on geriatric assessment and comprehensive primary care for our patient population. You will function independently as a certified nurse practitioner in collaboration with licensed physicians, practicing according to a written or electronic practice agreement.

Responsibilities

• Train new primary care providers in collaboration with the Regional Chief Clinical Officer on technology and EMR usage
• Provide bridging care in medical centers that are understaffed or need clinical coverage by seeing patients in clinic or virtually
• Address workflow items and return patient phone calls
• Independently assess acute and non-acute clinical problems
• Perform and document physical assessments and patient histories
• Analyze trends in patient conditions and develop, document, and implement patient management plans
• Assist in developing nursing plans of care and provide appropriate patient and family education and counseling
• Plan patient care based on in-depth knowledge of the specific patient population, anticipating physiological and psychological problems while considering cultural background, level of understanding, personality, and support systems
• Serve as a patient advocate
• Communicate with patients virtually or in clinic settings
• Order and interpret appropriate laboratory and diagnostic studies
• Order appropriate medications and treatments
• Refer patients for consultation when indicated
• Document in-depth progress notes and summaries
• Perform invasive procedures independently upon completion of documented competency
• Participate in patient care rounds and conferences
• Communicate patient management strategies to the patient care team
• Collaborate with multidisciplinary team members to ensure patient management strategies meet patient care needs
• Recognize situations requiring immediate physician attention and initiate life-saving procedures when necessary
• Use advanced communication skills to solve complex situations and improve processes and patient service
• Prescribe medication to patients based on state regulations
• Collaborate with multidisciplinary team members to analyze and evaluate current systems of health care delivery and implement new practice patterns as appropriate
• Participate in outside activities that enhance personal and professional growth and development
• Perform other duties as assigned and modified at the manager's discretion

Required Qualifications

• ARNP or similar advanced degree in Nursing
• Current Nurse Practitioner Certification in the State of practice
• Minimum 2 years of clinical experience as a Nurse Practitioner practicing in Family Medicine, Internal Medicine, or Geriatrics
• Past level of autonomy to make independent care decisions
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross (required within first 90 days of employment)
• Strong critical thinking and problem-solving skills
• Excellent communication and interpersonal skills
• Time management skills with the ability to work well under pressure
• Caring and empathetic with great listening skills
• Detail-oriented with the ability to pay close attention to patient charts, medications, and follow-up on patient care details
• Tech-savvy and proficient in Microsoft Office Suite and Electronic Medical Records
• Ability to exercise independent judgment
• Flexibility to work evenings, weekends, and/or holidays as needed

Preferred Qualifications

• Board certification by AANP or ANCC
• Experience as a preceptor or teacher

Additional Requirements

• Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
• This position may require 50-75% of local travel

Compensation

• Salary: $111,140 - $158,771 annually
• Final compensation depends on experience, education, geographic location, and other relevant factors
• This position may be eligible for bonuses or commissions
