Trumen Physicians + Associates is seeking a Nurse Practitioner to provide in-person clinical care to patients in skilled nursing and rehabilitation facilities across the Beaumont, TX area. In this independent contractor role, you will partner with Trumen-affiliated physicians and facility medical directors to manage chronic and acute conditions, support transitional care planning, and help prevent avoidable hospital readmissions.
Clinical Setting
You will work in skilled nursing facilities (SNFs) and rehabilitation settings, serving complex patient populations requiring post-acute and transitional care. You will maintain an on-site presence consistent with your agreed schedule and facility needs.
Responsibilities
- Collaborate with Trumen physicians and facility medical directors to coordinate patient care
- Conduct comprehensive clinical assessments and manage treatment plans
- Support transitional care planning to help prevent avoidable hospital readmissions
- Reconcile medications and provide patient education consistent with scope of practice
- Foster positive working relationships with facility leadership, clinical staff, and care coordinators
- Participate in Trumen-affiliated value-based and quality improvement initiatives as applicable
- Serve as a liaison between Trumen and designated skilled nursing facilities
- Maintain accurate and timely documentation within the EMR
Required Qualifications
- Active Nurse Practitioner license in the state of Texas
- Minimum of 1 year of experience as a Nurse Practitioner
- Ability to work independently while effectively collaborating with the care team
- Reliable transportation to assigned facilities
