Arkos Health is seeking a Nurse Practitioner to provide comprehensive clinical care to members with acute and chronic conditions in Casper, Wyoming and surrounding areas. This role combines in-home visits, virtual care, and in-clinic assessments to support complex and high-cost patient populations through a value-based care model that emphasizes quality, care coordination, and keeping patients healthy outside the hospital setting.

Clinical Setting

You will work in a collaborative environment that includes in-home visits at private residences and facilities, provider offices, and virtual care delivery. The position is recognized as an Advanced Practice Registered Nurse and Independent Licensed Practitioner by the Wyoming State Board of Nursing.

Responsibilities

  • Perform comprehensive in-home history and physical assessments that are complete, accurate, and appropriate for the member's age, gender, and clinical presentation
  • Conduct thorough reviews of available member records prior to scheduled visits to support accurate diagnosis capture
  • Utilize critical thinking to interpret diagnostic studies and screening evaluations
  • Develop, implement, evaluate, and modify treatment plans of care as needed
  • Make recommendations and adjustments to medications within scope of practice
  • Maintain accurate, complete, and timely documentation in the electronic health record, including correct diagnosis codes, status of acute and chronic conditions, and plans to address all identified diagnoses
  • Demonstrate understanding and working knowledge of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) models within 30 days of hire
  • Demonstrate understanding and working knowledge of Quality STAR/HEDIS measures within 30 days of hire
  • Participate in rotation of telephonic on-call coverage after hours

Required Qualifications

  • Current and valid State of Wyoming RN license
  • Bachelor's degree in Nursing
  • Wyoming Advanced Practice Nursing license
  • Board Certification from a nationally recognized certifying body for Nurse Practitioners
  • Unrestricted prescribing and dispensing privileges with DEA licensure
  • Minimum of 1 year of experience in an ambulatory or community care setting, or a combination of relevant work and clinical experience with strong assessment skills
  • Current driver's license and proof of insurance (driving is a requirement for this position)

Schedule

  • Monday through Friday with flexible scheduling
