Nurse Practitioner (APRN) – Home Health Support

The Lakes Home Care is seeking a licensed Nurse Practitioner (APRN) to support home health operations. In this role, you will play a key part in ensuring Medicare compliance, managing Plan of Care (POC) documentation, and conducting required Face-to-Face patient encounters with home health patients.

Responsibilities

• Review and sign Plan of Care (POC) documents and recertifications
• Perform Face-to-Face encounters as required by Medicare guidelines
• Support clinical documentation compliance with Medicare and AHCA regulations
• Collaborate with RNs, MDs, and administrative staff
• Assist in reducing hospital readmissions through patient follow-up

Required Qualifications

• Active Florida APRN license
• Active NPI number

Preferred Qualifications

• Medicare enrollment (PECOS)
• Experience in home health or geriatrics
• Knowledge of Medicare guidelines and documentation

Compensation and Schedule

• Per visit compensation (amount depends on service)
• POC review and signature compensation negotiable
• Monthly contract option available
• Flexible schedule
