Nurse Practitioner

We are seeking a Nurse Practitioner to join our healthcare team in a skilled nursing facility setting. You will work closely with physical therapy and a Physiatrist to provide comprehensive care focused on pain control and functional rehabilitation for patients in skilled nursing facilities.

Responsibilities

• Conduct thorough assessments of patients and identify and manage medical conditions under the supervision of a Physiatrist
• Develop and implement individualized care plans with a focus on function and rehabilitation to improve patients' quality of life
• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations
• Educate patients and their families on treatment plans, health maintenance, and preventive care
• Monitor and evaluate patient progress and adjust care plans as necessary to achieve optimal outcomes
• Submit accurate and thorough clinical documentation promptly in compliance with company policies and regulatory requirements

Clinical Setting

Skilled nursing facility with an interdisciplinary team including physical therapy and physiatry.

Required Qualifications

• Licensed Nurse Practitioner with valid Maryland certification
• Master's degree
• Ability to work independently while effectively managing your own caseload

Preferred Qualifications

• Two years of experience in a skilled nursing facility
