Nurse Practitioner - Albany, New York

WellBe Senior Medical is seeking a Nurse Practitioner to deliver comprehensive primary care to seniors in their homes throughout the Albany, New York area. This is a field-based position focused on value-based care, chronic disease management, and preventing unnecessary emergency department visits and hospitalizations.

Clinical Setting

You will conduct home visits to provide direct patient care, including comprehensive primary care assessments and physical examinations. You will manage a dedicated panel of high-risk patients, create longitudinal care plans, and address social determinants of health as part of a whole-person approach to care.

Responsibilities

• Conduct comprehensive primary care visits and physical examinations in patients' homes
• Manage chronic disease through evidence-based longitudinal care planning
• Treat acute medical needs in the home setting to prevent emergency department visits and hospitalizations
• Collaborate with an interdisciplinary care team to ensure safe care transitions and continuity of care
• Lead a sustainable patient panel with full support from the care team
• Exercise clinical autonomy in decision-making and problem-solving

Required Qualifications

• Master's Degree as a Nurse Practitioner
• Active, unencumbered NP license in New York
• Board Certification (ANCC or AANP)
• Valid driver's license and reliable vehicle for local field-based travel
• Legal authorization to work in the United States without employer sponsorship

Preferred Qualifications

• NP focus on Adult, Acute Care, Geriatric, and/or Family medicine

Compensation and Benefits

• Salary: $110,001 - $175,088
• Mileage reimbursement

Additional Requirements

Employment is contingent upon successful completion of a pre-employment drug screening and background check. As a healthcare organization, WellBe conducts monthly FACIS (Fraud and Abuse Control Information System) checks on all employees. Ongoing employment may be contingent upon compliance with the company's Drug-Free Workplace Policy, which includes random, post-accident, and reasonable suspicion drug testing.
