Nurse - Licensed Practical Nurse (LPN) - Mobile Medication - Warren, PA

Beacon Light Behavioral Health System is seeking a Licensed Practical Nurse (LPN) for its Mobile Medication Management program in Warren, PA. This community-based role provides in-home services to adults who experience difficulty progressing in their mental health recovery due to inconsistent adherence to their medication regimen. You will work directly with individuals and their prescribers to support self-management of medications and promote stability in their mental health treatment.

Clinical Setting

This is a mobile, community-based position. You will provide services in clients' homes and community settings rather than in a traditional clinical facility.

Responsibilities

• Provide in-home medication management services to adults with serious mental illness
• Work collaboratively with individuals and their prescribers to support medication adherence
• Support clients in self-management of their medications
• Coordinate care within the Mobile Medication Management program

Required Qualifications

• Nursing school certification or diploma
• Active LPN license

Preferred Qualifications

• Three years of nursing experience
• One year of psychiatric nursing experience serving seriously mentally ill adults

Schedule and Employment

• Full-time position
• Hourly employment

Compensation

• $18.38 - $21.24 per hour
• Retention bonus: $100 after 90 days for successful completion of probation; $250 after 6 months for completion of all training requirements and meeting position expectations
