Nurse, Home Health, Resource Pool

This is a resource pool home health nursing position providing professional nursing care to patients in their homes. You will utilize the nursing process—assessment, planning, nursing diagnosis, implementation, and evaluation—to deliver and document care according to medical plans, critical pathways, and organizational policies.

Responsibilities

• Provide and document professional nursing care in accordance with the medical plan of care and critical pathways
• Apply the nursing process to assess, plan, diagnose, implement, and evaluate patient care
• Coordinate and delegate care with other professionals and ancillary personnel
• Communicate findings to appropriate persons as indicated
• Perform additional job-related duties as requested by directors and managers

Schedule

• Monday through Friday, five 8-hour days
• Includes holiday and on-call rotation
