Nurse, Home Health, Full Time

This full-time home health nursing position provides professional nursing care to patients in their homes. You will utilize the nursing process—assessment, planning, nursing diagnosis, implementation, and evaluation—to deliver care according to established medical plans, critical pathways, and organizational policies.

Responsibilities

• Provide and document professional nursing care in accordance with the medical plan of care
• Assess patient conditions and develop appropriate nursing diagnoses
• Implement nursing interventions and evaluate patient outcomes
• Coordinate and delegate care with other professionals and ancillary personnel
• Communicate clinical findings to appropriate team members
• Perform additional job-related duties as requested by directors and managers

Schedule

• Monday through Friday, five 8-hour days per week
• Includes holiday and on-call rotation responsibilities
