NOW HIRING: Registered Nurse (RN) – Home Health

Uplift HomeCare is seeking a Registered Nurse (RN) to provide skilled in-home nursing care to patients in Gibsonton and surrounding areas of Florida. This PRN/part-time position offers flexible scheduling and the opportunity to support seniors and Veterans in maintaining their independence and receiving care in their homes.

Clinical Setting

You will deliver skilled nursing services in patients' homes across Polk, Hillsborough, and Pasco Counties, working with an interdisciplinary healthcare team including physicians, therapists, and other care coordinators.

Responsibilities

• Perform skilled nursing assessments and evaluate patient health status
• Develop, implement, and update individualized plans of care
• Provide nursing treatments and monitor patients for changes in condition
• Administer medications and treatments as ordered and within the RN's scope of practice
• Educate patients and caregivers on diagnoses, medications, safety, and disease management
• Coordinate care with physicians, therapists, and other members of the healthcare team
• Complete accurate, timely clinical documentation, including OASIS assessments when applicable
• Follow applicable state and federal regulations, infection-control practices, HIPAA requirements, and agency policies

Required Qualifications

• Current, unrestricted Florida Registered Nurse (RN) license
• Current CPR certification
• AHCA Level II background screening, as required
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical judgment, communication skills, organization, and attention to detail

Preferred Qualifications

• Home health nursing experience
• OASIS assessment experience
• Experience with Axxess or another home health electronic medical record (EMR)
