Now Hiring Full Time Home Health Registered Nurse RN

Halcyon Home, a central Texas-owned home health, hospice, and home care company, is seeking a full-time Home Health Registered Nurse to support our growing geriatric population in San Marcos and New Braunfels, TX. This role combines clinical assessment, care planning, and patient education in a home-based setting with an emphasis on compassionate, quality care for aging clients.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs, including risk for grief, cultural and spiritual considerations, and activities of daily living
• Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility
• Implement, develop, and document individualized plans of care; recommend revisions as necessary to ensure quality and continuity
• Provide effective communication with patients, families, team members, and other healthcare professionals
• Apply infection control and standard precautions in all patient care activities
• Provide clinical direction to hospice aides and LPN/LVN staff to ensure quality and continuity of service
• Prepare clinical and progress notes demonstrating progress toward established goals with appropriate documentation
• Inform physicians and other personnel of changes in patient needs and outcomes of interventions; evaluate patient and family response to care
• Monitor assigned cases to ensure compliance with third-party payor requirements
• Consult with and educate patients and families regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs
• Determine scope and frequency of services based on acuity and patient/family needs; assess caregiver ability to meet immediate patient needs
• Initiate appropriate preventive and rehabilitative nursing procedures

Required Qualifications

• Current RN license in good standing with the state nursing board
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively

Preferred Qualifications

• Experience in home health or related healthcare setting
• Knowledge or experience utilizing Homecare Homebase EMR
