New Graduate Licensed Practical Nurse (LPN) / Registered Nurse (RN)

Care Options for Kids is seeking a Licensed Practical Nurse (LPN) for pediatric home health nursing in Pennsylvania. In this role, you will provide one-on-one skilled nursing care to pediatric patients in their homes, working closely with a registered nurse and clinical leadership. Unlike traditional nursing settings, you will focus on a single patient per visit, allowing you to build meaningful relationships and deliver consistent, confident care.

Clinical Setting

You will work in pediatric home health, providing care in patients' homes rather than a hospital or clinic environment. This setting emphasizes focused, individualized patient care with clear plans of care developed and overseen by the RN team.

Responsibilities

• Provide skilled nursing care to pediatric patients with complex medical needs
• Support patients with conditions requiring G-tube, trach, or ventilator management
• Work collaboratively with registered nurses and clinical leadership
• Follow established plans of care with RN oversight
• Maintain adherence to CDC safety guidelines
• Utilize the employee portal to request shifts and manage scheduling preferences

Required Qualifications

• Valid Pennsylvania LPN License or Multistate License
• Valid BLS CPR card (obtained in person, not online)
• Valid driver's license
• TB Skin Test (PPD) or TB Blood Test (QF)
• Experience with or willingness to train in G-tube, trach, and vent care
