MDS Nurse - Registered Nurse

Berkley East Healthcare Center in Santa Monica is seeking an MDS Nurse to coordinate and assist with the completion and submission of accurate and timely patient assessments in accordance with all applicable laws, regulations, and company policies. This is a full-time, on-site position at a skilled nursing and rehabilitation facility serving short-term rehabilitation and long-term care patients throughout Los Angeles County.

Berkley East Healthcare Center is a skilled nursing facility focused on individualized care plans, advanced therapies, and meaningful outcomes. The facility is conveniently located near local hospitals and serves families throughout Los Angeles County.

Responsibilities

• Coordinate and assist with completion and submission of accurate, timely, inter-departmental patient assessments in accordance with all requirements and policies
• Report any changes in a patient's condition to the Director of Nursing (DON)
• Provide education to direct care associates regarding patient documentation, overseeing updates or changes, and training on Skilled Nursing Facility and other Regulations
• Ensure care plans are comprehensive, accurate, and meeting patient needs
• Review Final Validation Reports confirming assessments have been completed and accepted into the CMS and other systems as necessary and prior to billing
• Collaborate with Business Office regarding when to and when not to bill
• Continually review CMS Reports to address issues and improve processes to ensure that assessments are accurate and submitted on time
• Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings as assigned and requested
• Perform functions of a staff nurse when necessary
• Lead by example in treating everyone at work kindly and professionally, encouraging and reinforcing positive interactions

Required Qualifications

• Current state LVN license required; RN preferred
• Outstanding communication skills (verbal and written)
• Ability to remain kind, calm, and professional in all work-related situations
• Superior organizational skills and attention to detail

Preferred Qualifications

• Two years of nursing experience
• One year of MDS experience
• Takes pride in serving others and accurately completing duties on time

Compensation

• $55.00 - $65.00 per hour
