The opportunity

Job Description

Angel Care Hospice and Palliative is seeking an experienced Hospice and Home Health LVN to provide compassionate care for patients and their families in the Palmdale and Lancaster areas. You will work with patients residing in their own homes, assisted living facilities, and nursing homes throughout Southern California. This per diem position offers schedule flexibility to accommodate patient visits on weekly and biweekly frequencies.

You will work as part of a cohesive interdisciplinary team, collaborating with RN Case Managers and office support staff to ensure the highest quality of care during a difficult and emotional time for families.

Responsibilities

  • Coordinate with assigned patients to schedule visits at ordered frequencies
  • Communicate with the hospice team regarding patient needs
  • Provide education and support to patients and their families
  • Monitor patient response to interventions and adjust care plans as needed
  • Develop effective working relationships and cooperation with the hospice team throughout the entire case management process
  • Take initiative to interact with patients and advocate for their end-of-life goals
  • Maintain accurate and timely documentation of patient progress notes
  • Promote quality and cost-effective interventions and outcomes
  • Adhere to professional standards as outlined by protocols, rules, and regulations

Required Qualifications

  • Proven work experience in hospice care
  • Current LVN license
  • CPR certification
  • Valid driver's license and reliable transportation
  • Effective communication skills
  • Ability to work independently and as part of a team
  • Excellent organizational and time management skills
  • Strong clinical knowledge to manage complex patient needs

Preferred Qualifications

  • Experience with Hospice MD and Kinnser software for documentation
