LPN/RN Pediatric Home Health Nurse

Phoenix Home Care & Hospice is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide dedicated, one-on-one skilled nursing care to pediatric and young adult patients with specialized medical needs in their homes. This position is based in Lebanon, MO and surrounding areas, with weekday day shift scheduling.

In this role, you will deliver direct patient care according to physician-ordered plans, allowing patients with complex medical conditions to remain safe and supported at home. We welcome both experienced pediatric clinicians and nurses new to home care—comprehensive, hands-on clinical training is provided before your first independent shift.

Responsibilities

• Provide direct, one-on-one nursing care in accordance with the physician-ordered plan of care
• Administer medications, perform skilled treatments, and monitor vital signs and patient status
• Manage specialized medical equipment such as G-tubes, tracheostomies, or ventilators based on specific patient needs
• Maintain accurate, timely clinical documentation
• Collaborate closely with the patient's family, interdisciplinary care team, and clinical supervisors

Required Qualifications

• Current, unencumbered RN or LPN license in the state of practice
• Strong critical thinking, communication, and dependable follow-through

Preferred Qualifications

• Pediatric or home-care experience
