LPN/RN Pediatric Home Health Nurse

Phoenix Home Care & Hospice is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide dedicated, one-on-one skilled nursing care to pediatric and young adult patients with specialized medical needs in their homes. This position is based in Ava, Missouri and surrounding areas, with overnight shift availability. You will work in the Private Duty Nursing division, supporting patients to remain safe and cared for in their home environment.

We welcome both experienced pediatric clinicians and nurses new to specialized home care. Every nurse receives comprehensive, hands-on clinical training before taking their first independent shift.

Responsibilities

• Provide direct, one-on-one nursing care according to the physician-ordered plan of care
• Administer medications, perform skilled treatments, and monitor vital signs and patient status
• Manage specialized medical equipment such as G-tubes, tracheostomies, or ventilators based on specific patient needs
• Maintain accurate, timely clinical documentation
• Collaborate closely with the patient's family, interdisciplinary care team, and clinical supervisors

Required Qualifications

• Current, unencumbered RN or LPN license in the state of practice
• Strong critical thinking, communication, and dependable follow-through

Preferred Qualifications

• Pediatric or home-care experience

Schedule

• Overnight shifts with consistent, predictable scheduling options
