Home Care Network, Inc. is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide skilled nursing care to clients in the Youngstown, OH area (Mahoning County). This position focuses on wound care and private duty nursing (PDN) services, with a requirement to work weekends. You will have flexibility in choosing between part-time weekend-only hours or a part-time/full-time schedule that includes both weekends and weekdays.
Clinical Setting
You will deliver care in clients' homes, providing twice-daily wound care visits. Home Care Network, Inc. is a nurse-owned and locally led home health agency where patient care and wellbeing are prioritized. You will work under the direction of an RN Case Manager and report to the Clinical Administrator or Director of Nursing.
Position Options
Option 1: Part-Time Weekend Position
- Provide wound care visits twice daily on weekends
- Consistent patient assignment in the Youngstown area with no additional travel required unless you desire additional hours

Option 2: Part-Time or Full-Time Caseload Including Weekends and Weekdays
- Provide wound care visits twice daily on weekends in Youngstown, OH
- Create a part-time or full-time schedule serving additional clients on weekends and weekdays
- Travel up to 60 minutes for routine patient assignments
- Travel up to 90-120 minutes when necessary to maintain or build full-time hours
- Exposure to variety of patient populations and clinical experiences
Responsibilities
- Provide professional nursing care under the direction of a Registered Nurse
- Deliver skilled nursing visits for clients as directed by the RN Case Manager, following established care plans
- Perform wound care and related skilled nursing functions
- Assess client findings and monitor medications as appropriate to your license
- Administer medications within your scope of practice as delegated by the Case Manager
- Provide client teaching and emergency intervention as needed
- Recognize and report changes in client condition to the Case Manager and physician
- Communicate status updates verbally as needed and through complete, accurate documentation in the clinical record
- Promote personal safety and a safe environment for clients and coworkers
- Demonstrate knowledge of safety and infection control practices through compliance with policies and procedures
- Recognize and respond appropriately to potentially unsafe situations
- Demonstrate safe practice in equipment use
- Make recommendations for safety improvements
- Perform work in compliance with agency policies, procedures, and professional standards
- Follow applicable state and federal laws and agency standards
- Maintain confidentiality regarding client and staff information
- Participate in the Agency Quality Improvement Program
- Attend required meetings
- Perform other related duties as necessary
Required Qualifications
- Graduate of an accredited school of professional nursing
- Active LPN or RN license in good standing with current license to practice in the state of Ohio
- Minimum of 1 year of wound care experience
- Minimum of 1 year of direct patient experience with ventilator care
- At least 1 year in the home health industry plus 1 year of nursing experience
- Current driver's license or state ID
- Reliable transportation with mileage reimbursement provided between multiple clients
- Ability to work consistently and reliably on weekends in Youngstown, OH
- Comfortable working in homes with cats and dogs
- Willingness and ability to pass state and federal background checks
- Excellent attendance record with past employers
- Good interpersonal communication skills, both written and verbal
Preferred Qualifications
- Paid experience at a home health agency or equivalent
- Active CPR and First Aid certificates (agency will provide recertification if required)
- One or more years of experience with: Hoyer lift use and transfers, range of motion exercises, tracheostomy care, ventilator care, tube feedings and medication administration, respiratory treatments (nebulizer, shake vest, cough assist), colostomy care, catheter care, case management, PICC/PORT/IV care, lab draws, and OASIS and non-OASIS assessments
Schedule
- Weekends required
- Part-time weekend-only or full-time (weekends and weekdays) options available

Compensation
- LPNs: $28-$30 per hour
- RNs: $35-$40 per hour
- Compensation based on qualifications, certifications, experience, skill specializations, positive employment history, and overall client and team fit
- Weekly pay (Fridays) including onboarding and training
- Additional 4 hours on first paycheck after successfully completing first day of work
- Mileage reimbursement between multiple clients
