We are seeking a nurse for our Home Health department to provide high-quality nursing care to patients in their homes. This role requires strong clinical skills, excellent patient assessment abilities, and the capacity to work independently while collaborating with a multidisciplinary healthcare team.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed, including IV infusion and injections
- Monitor patient vital signs and report significant changes to the healthcare team
- Provide education to patients and families regarding health management and care plans
- Assist with activities of daily living, including tube feeding and catheter care
- Maintain accurate medical documentation in compliance with HIPAA regulations
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
- Implement infection control measures and ensure a safe environment for patients
- Support patients with developmental disabilities, Alzheimer's care, dementia care, and other specialized needs
- Participate in discharge planning and case management to facilitate smooth transitions of care
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice
- Ability to perform phlebotomy, venipuncture, catheterization, and other nursing procedures
- Excellent communication skills and ability to work effectively with diverse populations
Preferred Qualifications
- Experience in home health or community-based nursing
- Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth
- Strong clinical skills in critical care, geriatrics, pediatrics, acute care, or hospice care
- Familiarity with ICD-9/ICD-10 coding and medical terminology
- Knowledge of pain management techniques and acute pain management practices
- Experience working in hospitals (Level I/II trauma centers), nursing homes, assisted living facilities, or outpatient clinics
- Commitment to ongoing professional development and adherence to NCQA standards
