The opportunity

Job Description

We are seeking a nurse for our Home Health department to provide high-quality nursing care to patients in their homes. This role requires strong clinical skills, excellent patient assessment abilities, and the capacity to work independently while collaborating with a multidisciplinary healthcare team.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed, including IV infusion and injections
  • Monitor patient vital signs and report significant changes to the healthcare team
  • Provide education to patients and families regarding health management and care plans
  • Assist with activities of daily living, including tube feeding and catheter care
  • Maintain accurate medical documentation in compliance with HIPAA regulations
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
  • Implement infection control measures and ensure a safe environment for patients
  • Support patients with developmental disabilities, Alzheimer's care, dementia care, and other specialized needs
  • Participate in discharge planning and case management to facilitate smooth transitions of care

Required Qualifications

  • Current Registered Nurse (RN) license in the state of practice
  • Ability to perform phlebotomy, venipuncture, catheterization, and other nursing procedures
  • Excellent communication skills and ability to work effectively with diverse populations

Preferred Qualifications

  • Experience in home health or community-based nursing
  • Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth
  • Strong clinical skills in critical care, geriatrics, pediatrics, acute care, or hospice care
  • Familiarity with ICD-9/ICD-10 coding and medical terminology
  • Knowledge of pain management techniques and acute pain management practices
  • Experience working in hospitals (Level I/II trauma centers), nursing homes, assisted living facilities, or outpatient clinics
  • Commitment to ongoing professional development and adherence to NCQA standards
