(LPN) Licensed Practical Nurse PRN

WeCare at The Green Home Care Center is seeking Licensed Practical Nurses (LPN) on a PRN basis to provide essential nursing care to residents. This position is located at 37 Central Ave, Wellsboro, PA, 16901.

Responsibilities

• Conduct thorough assessments of each patient's medical status upon admission and throughout the course of treatment
• Provide regular patient status updates
• Assist in implementing the patient's treatment plan under the direction of the Registered Nurse
• Complete required documentation in an accurate and timely manner
• Supervise, administer, and/or assist with medications
• Provide direct care of residents as necessary
• Follow the plan of care for each resident
• Ensure resident care standards are maintained
• Report to the Director of Nursing

Required Qualifications

• Valid LPN license in the State of Pennsylvania
