LPN (Licensed Practical Nurse)

Mountain View Rehabilitation and Senior Living Center in Shamokin, Pennsylvania is seeking a full-time Licensed Practical Nurse (LPN) to provide direct patient care to residents in a long-term care, rehabilitation, and subacute setting. You will work with an experienced healthcare team dedicated to delivering high-quality clinical programs and services in a comfortable residential environment.

Responsibilities

• Provide direct patient care to residents in accordance with established policies and procedures
• Administer medications, treatments, and therapies as prescribed by physicians
• Monitor and report changes in resident condition to healthcare providers and family members
• Document resident care and treatment in accordance with facility policies and regulations
• Collaborate with healthcare team members to develop and implement individualized care plans
• Maintain a clean and safe environment for residents, including infection control and sanitizing procedures
• Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
