LPN - Licensed Practical Nurse

Maefair Center for Health & Rehabilitation, an affiliate of National Health Care Associates, is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a skilled nursing and rehabilitation setting. You will play a pivotal role in ensuring the well-being and comfort of our residents, working with an interdisciplinary team in a Certified Great Place to Work environment.

Clinical Setting

Maefair Center for Health & Rehabilitation is located at 21 Maefair Court, Trumbull, Connecticut, 06611. The facility provides short-term rehabilitation, skilled nursing, and post-hospital care for aging residents and those with chronic illnesses.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers
• Monitor and record patient vital signs and medical information
• Collaborate with the interdisciplinary team to develop and implement care plans
• Provide wound care and other nursing interventions as needed
• Supervise and support Certified Nurse Assistants (CNAs) in delivering care
• Ensure compliance with all regulatory standards and protocols

Required Qualifications

• Valid state LPN license
• Strong clinical skills including medication administration
• Ability to work with aging and chronically ill populations
• Excellent communication and interpersonal abilities
• Ability to work collaboratively in a multidisciplinary team environment
