Ashton Creek Health and Rehabilitation is hiring Licensed Practical Nurses (LPNs) for full-time positions in a long-term care setting. In this role, you will provide daily care, health assessments, and support to residents while contributing to a collaborative nursing team dedicated to helping residents reach and maintain their highest functional abilities.

Schedule

  • Full-time, 12-hour shifts (6:00 AM - 6:00 PM)

Responsibilities

  • Supervise, perform, and provide written and verbal care assignments and routine nursing skills and procedures to ensure quality resident care
  • Participate in the development and implementation of resident care plans, maintain up-to-date nursing documentation, and make recommendations for care plan revisions according to facility policy
  • Make daily and periodic visits to observe residents for symptoms and reactions to medications or treatments
  • Obtain and document vital signs, intake and output, weights, diet supplements and replacements, and cultures
  • Assist with the orientation and ongoing training of nursing staff to maintain required skill levels

Required Qualifications

  • Valid LPN license in good standing according to State and Federal requirements
  • Graduate of an accredited school of nursing
  • Thorough understanding of the principles of safe and effective nursing practices within the State of Indiana
  • Basic computer skills including knowledge of Excel and Word
  • Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and personnel
  • Ability to maintain confidentiality and protect the integrity of confidential information in compliance with Corporate Compliance and HIPAA requirements
  • Knowledge of personal computers, Word, Excel, and office machines such as fax and copying machines
