Ashton Creek Health and Rehabilitation is hiring Licensed Practical Nurses for full-time positions on 1st and 2nd shifts. In this role, you will provide daily care, health needs, safety, and comfort to residents while performing professional assessments and documentation of resident health.
Responsibilities
- Supervise, perform, and provide written and verbal care assignments, routine skills, and procedures to ensure quality resident care
- Participate in the development and implementation of resident care plans, maintain up-to-date nursing care plans, document progress, and make recommendations for care plan revisions according to facility policy and procedure
- Make daily and periodic visits to observe for symptoms, reactions to medication, and treatment responses
- Obtain and document vital signs, intake and output, weights, diet supplements and replacements, cultures, and other clinical data
- Assist with the orientation and ongoing training of staff to ensure required skill levels are maintained
Required Qualifications
- Graduate of an accredited school of nursing
- Valid LPN license in good standing according to State and Federal requirements
- Thorough understanding of the principles of safe and effective nursing practices within the State of Indiana
- Basic computer skills including knowledge of Excel and Word
- Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and personnel
- Ability to maintain discretion and protect the integrity of confidential information in compliance with Corporate Compliance and HIPAA
- Knowledge of personal computers, Word, Excel, and office machines such as fax and copying machines
Schedule and Shifts
- Full-time positions available
- 1st and 2nd shifts available
- $4 shift differential
