LPN Licensed Practical Nurse

Lock Haven Rehabilitation & Senior Living is seeking a Licensed Practical Nurse (LPN) to provide direct patient care at our facility located in Jersey Shore, PA. In this role, you will administer medications, perform wound care and other treatments, supervise Certified Nursing Assistants (CNAs), and collaborate with the interdisciplinary care team to deliver high-quality resident care.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and other treatments as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor patient vital signs and report any changes to the nursing team
• Supervise Certified Nursing Assistants (CNAs) and provide guidance and support
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Document patient care and treatment plans accurately and thoroughly
