LPN / Licensed Practical Nurse

Interim HealthCare is hiring Home Health Licensed Practical Nurses to provide patient care in Charlottesville and surrounding areas. In this role, you will deliver home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital discharge, working under the supervision of a Registered Nurse.

Responsibilities

• Provide direct home-based patient care to individuals with illnesses, injuries, and chronic diseases
• Take patient vitals, perform assessments, document progress, and report changes to the RN supervisor
• Assist the RN in carrying out the patient's plan of care and teaching patients self-care techniques
• Educate patients and families on the plan of care, medications, and home safety
• Collaborate with physicians, RNs, CNAs, aides, and the interdisciplinary team to coordinate patient care and plan for discharge

Required Qualifications

• Graduate of an accredited practical nursing program
• Active LPN license in Virginia
• Minimum of 2 years of LPN experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing, strong communication skills, and compassion
