Job Description
Immaculate Mary Center is hiring a Licensed Practical Nurse (LPN) to provide direct patient care in our residential facility. In this full-time position, you will work with our team-oriented staff to support the comfort and well-being of our residents.
Responsibilities
- Complete individualized patient care plans and perform care management and care coordination services
- Contact patients to provide care coordination, support, and manage compliance with care management programs
- Perform all responsibilities of the Licensed Practical Nurse role as assigned
Required Qualifications
- Active Pennsylvania LPN license
- Ability to adapt and take on all responsibilities of the Licensed Practical Nurse role
