Midland Care Connection is seeking a Licensed Practical Nurse (LPN) to provide hands-on nursing care for patients with terminal illnesses in our Hospice Home Care program. Working under the supervision of an RN, you will make routine visits to patients' homes and facilities to deliver compassionate, comfort-focused care that honors each individual's values and wishes during end-of-life care.
Responsibilities
- Administer medications and provide wound and catheter care
- Assist patients with hygiene, positioning, and activities of daily living
- Monitor pain and symptoms, observing for changes in patient condition
- Educate and support family caregivers, reinforcing care instructions and offering emotional reassurance
- Communicate patient needs and concerns to the hospice team to ensure care plans are adjusted promptly
- Work collaboratively with an interdisciplinary team including nurses, aides, social workers, chaplains, and volunteers
Schedule
- Full-time position, Monday through Friday, 8:00 AM to 5:00 PM
- Occasional on-call duties
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in Kansas
- Graduation from an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification
- Valid driver's license, reliable transportation, and proof of auto insurance
- Ability to provide direct patient care under RN supervision
- Strong assessment and observation skills with ability to report changes in patient condition
- Effective communication skills with patients, families, and interdisciplinary team members
- Ability to work independently in home and facility settings
- Physical ability to perform job duties, including lifting, bending, and prolonged standing
Preferred Qualifications

- Sign-on bonus of $2,500, paid in two increments at 6 months and 1 year of employment
