LPN Licensed Practical Nurse - Home Health/Hospice - Per Diem

This per diem position offers flexible home health and hospice nursing work under the supervision of a Registered Nurse. You will deliver direct patient care based on physician orders, manage patient revisits, and participate in multidisciplinary case conferences. This role requires strong time management, clinical competence, and the ability to work independently while collaborating with the healthcare team.

Responsibilities

• Deliver nursing care to patients as an associate nurse under RN supervision
• Complete patient revisits and participate in multidisciplinary case conferences
• Manage time effectively, demonstrate flexibility, and prioritize patient care needs
• Perform nursing skills competently and within scope of practice
• Maintain current knowledge of federal, state, and agency documentation requirements and ensure timely submission
• Collaborate with the RN, Director of Patient Services, patients, and other healthcare providers in planning patient care
• Communicate patient observations clearly to fellow healthcare providers
• Maintain respectful, collaborative relationships with all members of the healthcare team
• Perform other duties as assigned

Required Qualifications

• High school diploma or equivalent
• Graduate of an approved practical nursing course
• Current state license as a Licensed Practical Nurse

Preferred Qualifications

• One year of experience in home care or public health nursing

• Travel is always required for this position

Compensation

• $38.96 per hour (contract-controlled rate)
