LPN (Licensed Practical Nurse) Home Health

Phoenix Home Health is seeking a Licensed Practical Nurse (LPN) for full-time home health care in Jefferson City, Missouri and surrounding areas. In this role, you will serve as the vital link between patients and their medical providers, delivering skilled nursing care in patients' homes.

You will execute tailored care plans based on physician orders while incorporating insights from initial assessments. Your work will include physical assessments, patient instruction, and ongoing communication with patients and their families. You will collaborate closely with an interdisciplinary team including fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.

Responsibilities

• Assist in review and revision of the plan of care
• Provide nursing care in accordance with the physician's plan of treatment and individual plan of care as authorized by the client's payer source
• Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician
• Provide ongoing instruction to clients and families regarding the plan of treatment
• Participate in education programs, quality assessment activities, and staff meetings as required

Schedule

• Monday through Friday, 8:00 AM to 5:00 PM
• Weekend rotation required every 4 to 6 weeks

Required Qualifications

• Missouri or Compact Registered Nursing or Licensed Practical Nursing License
• Valid driver's license
• Strong organizational and self-management skills
• Strong and compassionate customer service skills

Compensation and Benefits

• Pay Range: $50,000 - $80,000
