LPN (Licensed Practical Nurse) Home Health

Buckeye Home Health Care of Cincinnati is seeking a Licensed Practical Nurse (LPN) to deliver compassionate, personalized home health care to patients in the West Chester, Mason, Fairfield, and surrounding areas. In this full-time role, you will work independently in patients' homes, executing physician-directed plans of care while maintaining close communication with the clinical team to optimize patient outcomes and prevent hospital readmissions.

Responsibilities

• Deliver personalized plans of care directed by physicians to keep patients safe and comfortable at home
• Monitor patient condition changes and adjust care plans early to minimize hospital visits and re-hospitalizations
• Communicate directly with physicians regarding medication needs and ongoing care adjustments
• Teach and counsel patients and family members on managing injuries, diseases, and recovery processes
• Complete patient care records efficiently using the WellSky EMR system
• Attend orientation, ongoing staff meetings, and earn continuing education units

Required Qualifications

• Active Ohio LPN license in good standing
• Current CPR certification
• Reliable transportation and current auto insurance
• Solid computer skills and ability to navigate an EMR system
• Working understanding of Medicare and Medicaid guidelines for home health
• Strong independent work ethic

Preferred Qualifications

• Experience with WellSky EMR system
