LPN Licensed Practical Nurse

Hamilton Grove Healthcare and Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to provide basic nursing care to residents in our healthcare and rehabilitation facility. In this role, you will be a vital member of our interdisciplinary healthcare team, delivering compassionate patient care in a warm and welcoming environment.

Responsibilities

• Provide basic nursing care to patients, including administering medications, performing vital signs, and assisting with activities of daily living
• Conduct routine vital sign checks and document patient information accurately
• Assist with patient transfers and ambulation as needed
• Develop and implement personalized care plans in collaboration with the healthcare team
• Provide education and support to patients and their families
• Participate in quality improvement initiatives to ensure high-quality patient care
