Responsibilities

• Administer medications and treatments as ordered by physicians
• Monitor and report changes in residents' conditions
• Maintain accurate and up-to-date documentation of resident care
• Assist with admissions, discharges, and transfers
• Collaborate with interdisciplinary team members to implement individualized care plans
• Maintain a clean, safe, and organized environment while following infection control procedures
• Promote a positive and supportive atmosphere for residents, families, and staff

Required Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
• Strong communication and interpersonal skills
• Ability to work collaboratively with residents, families, and the healthcare team
