LPN Licensed Practical Nurse

Evercare of Collinsville is seeking a Licensed Practical Nurse to provide direct patient care under the supervision of a registered nurse. You will work in a fast-paced healthcare environment, delivering high-quality care and contributing to the overall success of the facility.

Responsibilities

• Provide direct patient care under RN supervision, including administering medications, wound care, and vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report patient progress, including any changes or concerns
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
• Participate in quality improvement initiatives and educational programs to stay current with best practices

Required Qualifications

• Current LPN licensure in the state of Illinois
• Ability to work in a fast-paced environment with frequent interruptions
• Ability to lift, transfer, and reposition patients as needed
• Excellent communication and interpersonal skills

Experience

• Minimum of 0 years of experience as an LPN. Recent graduates are encouraged to apply.
