LPN (Licensed Practical Nurse) Evening shift

Valley Manor Rehabilitation and Healthcare Center in Coopersburg, Pennsylvania is seeking a Licensed Practical Nurse (LPN) for the evening shift. This 180-bed rehabilitation and healthcare facility provides care to patients in recovery, and you will play a vital role in delivering high-quality patient care and support to residents.

Responsibilities

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
• Conduct thorough patient assessments and develop individualized care plans in collaboration with the healthcare team
• Implement and monitor treatment plans, ensuring timely and effective patient outcomes
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Participate in patient education and support, promoting independence and empowerment
• Maintain accurate and timely records, adhering to facility and regulatory standards

Required Qualifications

• Current LPN licensure in the state of Pennsylvania
• Ability to work varied shifts, including days, evenings, and weekends
• Strong communication and interpersonal skills
• Ability to work effectively as part of a team
