LPN Licensed Practical Nurse

Charter Senior Living is seeking a Licensed Practical Nurse (LPN) to provide direct care support to residents in our Assisted Living and Memory Care programs. In this role, you will be a main support provider, assisting residents through assessments and care planning, medication and treatment management, implementation and evaluation of resident plans of care, and documentation of nursing services. You will enable residents to lead a quality life while maintaining their dignity, working with patience and a collaborative spirit alongside our care team.

Responsibilities

• Provide direct care and support to residents in Assisted Living and Memory Care programs
• Conduct resident assessments and participate in care planning
• Manage medications and treatments according to care plans
• Implement and evaluate resident plans of care
• Document nursing services and resident care activities
• Collaborate with interdisciplinary team members
• Support residents in maintaining dignity and quality of life
