LPN / Licensed Practical Nurse - Case Management

All Ways Caring HomeCare is seeking an LPN/Licensed Practical Nurse for a Case Management position. In this role, you will serve as a key operational team member responsible for managing the intake and pre-screening process for clients seeking home care services. You will work with referral sources, potential clients, and third-party payers to assess client needs, determine service feasibility, and facilitate case acceptance and placement.

Responsibilities

• Receive and respond to incoming calls from referral sources and potential clients to identify needs and assess the company's ability to meet them
• Conduct pre-screening assessments and prepare pre-screening reports and case proposals
• Complete pre-admission paperwork including obtaining payment authorization and coordinating treatment team availability
• Oversee and direct field staff assisting with the pre-screening process
• Access and review national, state, and company account information, including contract terms and past payer agreements
• Consult with third-party representatives regarding client benefit coverage, financial responsibility, service authorization, and reimbursement procedures
• Present company services and negotiate reimbursement levels with third-party payers
• Contact referral sources to advise of case acceptance and provide information on assigned clinical teams
• Develop and maintain knowledge of all company services and community resources
• Identify alternative community service sources when company solutions are not appropriate or available
• Maintain relationships with referral sources and payer case managers
• Track referral source satisfaction levels, conversion ratios, and provide summary reports to management
• Implement and maintain operational processes to ensure compliance with company policies and regulatory mandates
• Adhere to HIPAA privacy practices and Business Ethics and Compliance programs
• Participate in quarterly growth planning meetings and discussions regarding staffing and recruitment
• Perform other duties as assigned

Clinical Setting

All Ways Caring HomeCare provides home-based care services including personal care, homemaking, professional nursing, older adult care management, Alzheimer's/dementia care, and respite care. You will work with clients of all ages recovering from illness, injury, or surgery, living with chronic disabilities, or experiencing the natural process of aging.

Required Qualifications

• Bachelor's degree in a human services field or nursing field
• Valid driver's license
• Two years of experience with case management or related discipline
• Ability to communicate both verbally and in writing

Compensation

USD $22.00 - $24.00 per hour
