LPN Licensed Practical Nurse

Belmont Village Senior Living is seeking a Licensed Practical Nurse to provide medication administration, clinical oversight, and care coordination in our Assisted Living and Memory Care communities. This full-time or part-time position offers the opportunity to work with seniors in a fast-paced, hands-on environment while leading and supporting our care team.

Clinical Setting

You will work in an Assisted Living and Memory Care community, collaborating with healthcare practitioners, ancillary providers, pharmacies, family members, and team members to ensure timely clinical interventions and quality resident care.

Responsibilities

• Administer medications to assigned residents according to physicians' orders and report any adverse reactions or clinical observations
• Follow up on physicians' orders and coordinate services with ancillary providers
• Report to physicians and families any changes in resident condition or other pertinent clinical care information
• Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers
• Make appropriate situational decisions and manage conflict in a positive and constructive manner
• Update and review resident medical records, clinical documentation, and incident reports

Schedule

• Full-time and part-time positions available
• Hours: 6:30 AM to 7:00 PM
• Rotation includes every other weekend and two weekdays
• Weekend availability is required

Required Qualifications

• Active LPN license in the state where the community is located
• CPR certification or ability and willingness to obtain within 30 days of hire
• Ability to communicate clearly in verbal and written English
• Intermediate computer and mobile device skills
• Professional, pleasant, and team-oriented attitude
• Consistent and reliable attendance

Preferred Qualifications

• One year of medication administration experience
