Elite Care - Oatfield Estates is seeking a Licensed Practical Nurse to join our residential care team in Milwaukie, Oregon. As an LPN, you will promote the health and well-being of residents by providing nursing assessments, contributing to care plan development, and educating staff on appropriate resident care. You will help develop systems, procedures, and training programs that support state and federal regulations.
Clinical Setting

Responsibilities
- Develop and support communication systems with the Director of Nursing regarding resident and family concerns, changes in resident condition, employee injuries, budget issues, and operational matters
- Review and perform evaluations of resident health and functional care needs before move-in to ensure the community can meet resident needs and meet regulatory requirements
- Complete resident assessments before move-in, when changes in condition occur, and as required by company policy and state regulation
- Assume on-call responsibilities on a rotational basis and assign on-call duties to other qualified management
- Develop knowledge of community policies, procedures, and state and federal regulations pertaining to resident care
- Perform selected clinical tasks that reflect unusual or unique resident conditions or cannot be delegated, such as IM injections
- Monitor resident health status and initiate actions in response to changing needs
- Assist in coordinating resident transfers to or from hospitals or other health facilities
Required Qualifications
- Active Oregon-endorsed LPN license
- Minimum 1 year of LPN experience in a related field or setting
- Ability to pass an Oregon background check
- Strong knowledge of Oregon Board of Nursing requirements, especially as they pertain to nursing delegation
- Strong ability to teach and train on all components of nurse delegation
- Thorough knowledge of medication room components and medication administration
- Reliable transportation
- Must be 18 years or older
- Must be a U.S. Citizen and reside within the State of Oregon
Schedule
Shift options include Thursday-Monday, 3 p.m.–11 p.m. or Tuesday-Saturday, 8-hour shift. Shift days are flexible, but weekend availability is required.

- Compensation: $40.00 to $45.00 per hour (based on experience)
