LPN – Licensed Practical Nurse Baylor Day shift

Millennium Post Acute Rehabilitation, a 132-bed skilled nursing and rehabilitation facility in West Columbia, SC, is hiring full-time Licensed Practical Nurses for 12-hour day shifts. In this role, you will provide direct patient care in a post-acute setting, supporting residents' recovery and daily needs while working collaboratively with an interdisciplinary nursing team.

Responsibilities

• Provide direct patient care and monitor resident conditions
• Administer medications and treatments per physician orders
• Perform routine procedures including vital signs, injections, and wound care
• Document accurately in patient charts
• Communicate changes in resident condition to physicians and the nursing team
• Collaborate on individualized care plans
• Support residents' daily needs in a skilled nursing and post-acute setting

Schedule

• 12-hour day shifts
• Full-time and part-time positions available

Required Qualifications

• Active South Carolina LPN license
• Ability to work 12-hour shifts
• Strong clinical and documentation skills

Preferred Qualifications

• Experience in skilled nursing, long-term care, or post-acute settings
