LPN (Licensed Practical Nurse)

Adamsville Healthcare and Rehabilitation Center is seeking Licensed Practical Nurses to join our care team on the night shift. In this role, you will provide direct care to residents in our long-term care facility, working as part of a collaborative nursing team to support resident health, safety, and quality of life.

Clinical Setting

Adamsville Healthcare and Rehabilitation Center is a long-term care facility serving residents who require ongoing medical support and rehabilitation services.

Responsibilities

• Direct the daily activities of CNAs and ensure nursing personnel follow policies and procedures
• Participate in admissions, discharges, and transfers of residents as required
• Assess resident conditions and prepare and administer medications per physician orders
• Initiate, review, and update resident care plans
• Report changes in resident condition to the nurse supervisor and communicate factors that may affect your ability to perform assigned work
• Perform other duties as assigned by clinical or administrative leadership
• Maintain confidentiality of all protected health information in verbal, written, and electronic communications

Schedule

• Night shift position
• Full-time employment

Required Qualifications

• Current unrestricted LPN license for the state of Tennessee
• Current CPR/BLS certification
