Chapters Health System is seeking a Licensed Practical Nurse (LPN) to provide nursing interventions and patient teaching in a field-based setting, working Monday through Friday. Under RN supervision, you will deliver direct patient care, manage symptoms, perform clinical procedures, and enhance quality of life for patients and their families in a home or community-based environment.
Clinical Setting
This position serves patients in various settings including home care and PACE (Programs of All-Inclusive Care for the Elderly) programs. You may work with patients experiencing chronic illness, terminal conditions, and age-related conditions including Alzheimer's Disease and related dementias.
Responsibilities
- Provide nursing interventions and patient/family teaching under RN direction
- Perform continuous patient care, dressing changes, and catheter changes as identified in the plan of care
- Observe and monitor patients for alterations in body systems, pain, and symptoms; report findings to the RN
- Observe and report signs of adverse reactions and responses to treatments
- Report clinical findings to physicians, APRNs, and physician's assistants following collaboration with the RN
- Perform venipuncture for blood specimens
- Perform Central Venous Line (CVL) site observation and inspection; manage venous access device dressing, cap changes, and flushing; administer IV medications and fluids; perform IV pushes per procedure; and perform venipuncture (note: you may not make changes to infusion pumps, including rate changes or IV bag changes)
- Document care per policy and procedure in the medical record
- Perform other duties as assigned
- During emergencies such as hurricanes, you may be required to report to a company-designated location to ensure continuity of services, which may include reporting ahead of your scheduled date/time and staying overnight based on the duration of the emergency
Schedule
Monday through Friday. A flexible schedule is required to meet patient needs.
Required Qualifications
- High school diploma or G.E.D. or equivalent combination of education and experience
- Current LPN licensure in the state of employment
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Valid driver's license and automobile insurance per company policy
- Reliable transportation to meet visit schedule
- Ability to communicate in written and verbal English
- Ability to use equipment with visual and auditory mechanisms
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
- Ability to perform essential functions and physical requirements of the job, including lifting patients and/or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation
- Ability to work within scope of practice per state regulations
Certification Requirements
- Employees working at PACE locations must have certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs
- For current Hope Employees: BLS must be obtained by January 1, 2025
- For current Capital Caring employees: BLS must be obtained by November 1, 2024
- If working in an inpatient unit: certificate of completion of 30-hour Infusion Therapy Course and in-house 8-hour Infusion Class (preferred)
Preferred Qualifications
- Certificate of completion of 30-hour Infusion Therapy Course and in-house 8-hour Infusion Class for inpatient unit positions
- Cross-training capability for Hospice House positions of equal licensure/certification and scope
Compensation
$22.30 - $33.18 per hour

- This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug-Free Workplace Policy
- All employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made and for ongoing eligibility verification
- Some locations may require cross-training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing
