This on-site contract position offers an excellent opportunity for an associate-level Nurse Practitioner or Physician's Assistant to deliver high-quality patient care in a community-focused primary care practice. You will work directly with patients to diagnose, treat, and manage a wide range of acute and chronic conditions, collaborating with physicians and other healthcare professionals to ensure comprehensive, patient-centered care.

Clinical Setting

You will practice in a supportive, community-focused primary care environment serving patients ages 18 and older, with a specialization in geriatric care.

Responsibilities

  • Conduct comprehensive patient assessments, including medical histories and physical examinations
  • Diagnose and treat acute and chronic illnesses common in family medicine, internal medicine, and primary care
  • Develop and implement individualized patient care plans based on clinical findings and patient needs
  • Order and interpret diagnostic tests, including laboratory work, imaging, and other procedures
  • Prescribe medications, therapies, and other treatments within the scope of practice
  • Provide patient education on health maintenance, disease prevention, lifestyle modifications, and treatment adherence
  • Perform routine physical exams and health screenings
  • Manage sick visits and same-day appointments
  • Administer immunizations and manage medication prescriptions and refills
  • Collaborate with physicians, nurses, and interdisciplinary healthcare teams to ensure coordinated care
  • Maintain accurate, timely, and confidential electronic medical records (EMR)
  • Participate in quality improvement initiatives and clinical protocol development within the practice
  • Ensure compliance with all regulatory requirements, clinical guidelines, and standards of care

Required Qualifications

  • Master's degree in Nursing (NP) or Physician Assistant Studies (PA) from an accredited program
  • Active licensure as a Nurse Practitioner or Physician's Assistant in New York, or the ability to obtain licensure
  • Board certification in Family Medicine, Internal Medicine, or Primary Care
  • Associate-level clinical experience, including clinical rotations, residency training, or prior locum assignments
  • Strong clinical assessment, diagnostic, and critical-thinking skills
  • Excellent written and verbal communication skills for effective patient and team interaction
  • Ability to work autonomously and collaboratively within a multidisciplinary healthcare team
  • Proficiency with electronic medical record (EMR) systems and telehealth tools
  • Current BLS and ACLS certifications

Schedule

  • Monday through Friday, 8:00 AM to 5:00 PM
