Licensed Vocational / Practical Nurse- Home Health

Elara Caring is seeking a Licensed Vocational/Practical Nurse to deliver compassionate, high-quality care to patients in their homes. In this role, you will work as part of an interdisciplinary care team to support positive patient outcomes, help prevent avoidable hospitalizations, and enable patients and caregivers to manage their health effectively.

Responsibilities

• Follow each patient's individualized care plan and physician orders
• Monitor patient progress and report changes or safety concerns to the care team
• Administer and document medications according to agency guidelines
• Help patients and caregivers learn self-care techniques to support their health
• Work with the RN Case Manager to update care plans as patient needs change
• Communicate with physicians, patients, families, and the care team to ensure coordinated care
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care

Required Qualifications

• Current State License as a Licensed Vocational/Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• One year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Preferred Qualifications

• Experience in a hospice or home health environment
