Licensed Vocational Nurse LVN PRN Home Health

Elara Caring is seeking a Licensed Vocational Nurse (LVN) for a PRN home health position. In this role, you will provide direct patient care in patients' homes, following individualized care plans and physician orders while monitoring patient progress and communicating with the care team to ensure coordinated, compassionate care.

Responsibilities

• Follow each patient's individualized care plan and physician orders
• Monitor patient progress and report changes or safety concerns to the care team
• Help patients and caregivers learn self-care techniques to support their health
• Work with the RN Case Manager to update care plans as patient needs change
• Document medication regimens and update medication profiles according to agency guidelines
• Communicate with physicians, patients, families, and the care team to ensure coordinated care
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care
• Deliver high-quality care that supports positive patient outcomes

Required Qualifications

• Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• One year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations

Preferred Qualifications

• Experience in a hospice or home health environment
