Licensed Vocational Nurse (LVN) - Home Health - West LA

GrandCare Health Services is seeking a Licensed Vocational Nurse (LVN) to provide home health care to patients recovering from surgery, illness, and injury in the South Orange County area. This part-time position specializes in post-surgical and orthopedic rehabilitation, offering one-on-one patient care in patients' homes with flexible scheduling and autonomy.

As an LVN, you will work under the supervision of a Registered Nurse and follow established plans of care to deliver high-quality nursing services and patient education. This role is ideal for nurses who enjoy building relationships with patients and supporting their recovery journey in a home setting.

Responsibilities

• Deliver nursing care as directed by the RN and patient plan of care
• Provide accurate, timely, and compliant documentation
• Collect assessment data and communicate findings to RN and physicians
• Collaborate with the interdisciplinary care team
• Use clinical equipment safely and efficiently
• Engage in ongoing professional development

Required Qualifications

• Active California LVN License
• Current CPR Certification
• 2 or more years of direct patient care experience
• Home health experience required
• Valid driver's license, reliable insured vehicle, and good driving record
• Willingness to bathe patients and experience with wound, foley, and ostomy care

Compensation and Benefits

• Per visit pay: $45.00 to $48.00 per completed visit (based on experience)
• Mileage reimbursement
• Paid drive time
• Documentation and productivity bonuses
• Annual salary range: $60,000 to $90,000 (based on pay per visit model)
