Life Care Home Health is seeking a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. This is a PRN (as-needed) position serving patients across Texas, Florida, Nevada, and Georgia. You will work under the direction of a Registered Nurse to deliver quality patient care, coordinate with interdisciplinary teams, and support patients and families in maintaining health and preventing illness relapse.
Clinical Setting
You will provide care in patients' homes in various conditions, working with patients of all ages. You may be exposed to blood-borne pathogens, bodily fluids, and infectious diseases. Some exposure to unpleasant weather is possible, and PRN emergency call availability is required.
Responsibilities
- Administer nursing care under the supervision of a Registered Nurse
- Assist in identifying clients' physical, psychosocial, and environmental needs through documentation review, clinical records, case conferences, and team reports
- Participate in planning and implementing care in accordance with the Plan of Care (POC)
- Assist clients in recovery or maintenance of life functions and prevent illness relapse
- Provide care utilizing infection control measures that protect both client and staff (OSHA compliance)
- Ensure continuity of care through delivery of quality patient care
- Communicate effectively with clients, families, team members, and other healthcare professionals
- Participate regularly in case conferences and team meetings
- Monitor assigned cases to ensure compliance with third-party payer requirements
- Complete appropriate documentation in a timely manner per company policy
- Participate in in-service programs and maintain professional competency
- Meet mandatory continuing education requirements of the Agency and licensing board
Required Qualifications
- Completion of an accredited Licensed Vocational Nursing or Licensed Practical Nursing program
- Current LVN/LPN license in good standing in any applicable state
- Current CPR certificate
- Current driver's license in good standing
- One year of experience as an LVN/LPN in a clinical care setting
- Ability to read, write, and comprehend English
- Nursing skills as defined by generally accepted standards of practice
- Excellent interpersonal skills and ability to communicate effectively
- Proven decision-making skills
- Hepatitis profile (if required to make visits)
- Reliable transportation and auto liability insurance

Schedule and Travel
This is a PRN position requiring flexible scheduling. You must be able to travel locally to patients' homes and be available for emergency calls. A personally owned vehicle with current auto liability insurance is required.
Equipment
You will need to provide or have access to a manual blood pressure cuff, stethoscope, and thermometer.
Licensing and Certification Maintenance
Ongoing employment requires maintaining a valid LVN/LPN license in the state where you practice and a current CPR certificate at all times. You are responsible for renewing both credentials before they expire to continue employment.
