Licensed Vocational Nurse (LVN) - Home Health

Transition Home Healthcare is seeking a Licensed Vocational Nurse (LVN) to provide home health nursing care in the Southeast Houston area. In this full-time position, you will deliver direct patient care in patients' homes, working collaboratively with the interdisciplinary home healthcare team to develop and implement individualized care plans.

Responsibilities

• Complete home care nursing visits
• Collaborate with the home healthcare team to develop and implement individualized care plans
• Prepare timely, complete, and accurate clinical notes
• Communicate with healthcare providers and other care team members
• Educate patients and their families about health conditions, treatment plans, and self-care techniques
• Adhere to all agency policies, procedures, and regulatory requirements related to home health care

Required Qualifications

• Valid and current LVN or LPN license in the state of Texas or compact state
• Minimum of one year of experience in a home health setting
• Knowledge of home health regulations and general healthcare compliance
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members

Preferred Qualifications

• Experience with HCHB EMR
