Licensed Vocational Nurse (LVN) Home Health

Health Link Home Health and Hospice is seeking a Full-Time Licensed Vocational Nurse (LVN) to provide home health services to patients throughout San Francisco. In this role, you will implement plans of care, assess and educate patients and caregivers, coordinate with the interdisciplinary team, and maintain accurate documentation. You will work collaboratively with the Director of Nursing, clinical staff, and office team to deliver coordinated, high-quality care.

Clinical Setting

This is a home-based nursing position serving patients across the greater San Francisco Bay Area. You will travel to patient homes to provide direct care and coordinate services with other disciplines including physical therapy, occupational therapy, speech therapy, and social work.

Responsibilities

• Implement the plan of care initiated by the Registered Nurse
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
• Provide accurate and timely documentation consistent with the plan of care
• Complete online charting in Kinnser using company-provided devices
• Participate in coordination of home health services and report additional needs to the RN or Clinical Supervisor
• Use equipment and supplies effectively and responsibly
• Follow all company policies and procedures
• Perform other duties as assigned

Required Qualifications

• Active California LVN license
• Valid California Driver's License
• Minimum 1 year of nursing experience in a medical setting
• IV certification
• Experience with wound care

Preferred Qualifications

• Home health experience

Schedule and Employment

This is a full-time position, not a contract role. The schedule offers flexibility with low production goals to allow quality patient care.

Compensation and Benefits

Pay: $93,000 - $115,000 per year, depending on experience. Overtime eligibility is available; LVNs are not exempt from California overtime laws.

Bonuses are based on quality metrics, not patient volume.

The company provides an iPad and all necessary tools for efficient documentation and communication, including AI charting software to reduce time spent on documentation.
