TheKey Concierge Nursing Services is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care to clients in their homes throughout Los Angeles, CA. In this per diem/on-call position, you will deliver comprehensive 1:1 patient care, conduct nursing assessments, develop individualized care plans, and serve as a trusted point of contact for clients and their families. This role combines direct clinical care with care coordination and client retention responsibilities in a home-based setting.
Clinical Setting
You will work in clients' homes, providing skilled nursing services and health education. The role includes both in-person and virtual reassessments to support clients' physical, emotional, and social well-being while promoting independence and quality of life.
Responsibilities
- Proactively connect with new clients immediately upon assignment to schedule initial meetings and establish collaborative relationships with clients and their families
- Conduct comprehensive, in-person initial nursing assessments, reassessments, and quality assurance visits, including home safety evaluations, daily routines, preferences, and formal/informal support systems
- Develop individualized care plans that incorporate preventative and rehabilitative nursing procedures tailored to each client's unique needs
- Deliver skilled nursing services in full accordance with established plans of treatment
- Oversee and coordinate all care and services provided to ensure a seamless client experience
- Perform regular home visits and timely reassessments during condition changes, updating care plans and communicating promptly with physicians, practitioners, interdisciplinary team members, and the agency
- Provide ongoing instruction and health education to clients, family members, and care staff
- Serve as a highly visible, trusted point of contact to maintain an exceptional experience for clients and their support networks
- Drive long-term retention through dedicated hands-on attention, client appreciation initiatives, and proactive communication
- Provide dedicated assistance and close oversight during the launch of new client cases
- Maintain full compliance with state home care regulations, agency standards, and visit documentation requirements
- Prepare accurate clinical notes and submit documentation promptly according to agency policy
- Respond to inquiries, calls, and messages within agency-established timeframes
- Monitor and review client service rates as care needs evolve
- Provide on-call and after-hours support as required
- Perform other duties as assigned to support clinical and business goals
Required Qualifications
- Current, unencumbered Licensed Vocational Nurse (LVN) license in California with active status maintained as a condition of employment
- Minimum 2 years of experience in healthcare, eldercare, home care, or a related clinical field
- Computer proficiency with the ability to complete and submit accurate, timely clinical documentation in the electronic health record system
- Strong customer service, communication, and conflict-resolution abilities
- Valid driver's license and proof of current auto insurance
Schedule and Compensation
- Position Type: Per Diem / On-Call / Part Time
- Starting Compensation: $40/hour (varies by tier)
- On-call and after-hours availability required
